TikTok is the channel chiropractors ask us about most and run worst. The platform rewards exactly what a chiropractic clinic has in abundance: visible, satisfying, slightly dramatic treatment footage. Yet most clinic accounts we audit either post organically and hope, or copy their Facebook campaign into TikTok Ads Manager and wonder why the cost per booked visit is double. This guide is the setup we run for clinics in Spain, the US and Latin America, with the numbers we actually see.
Adjustment videos are among the most-watched health content on the platform. That matters because TikTok's ad auction prices attention by watch time and completion: a clinic whose 15-second clip gets watched to the end pays a fraction of the CPM of a generic "book now" ad. In our clinic accounts TikTok CPMs run between €4 and €9, roughly half of what the same clinics pay on Meta.
The limitation is intent. Nobody opens TikTok with back pain looking for a chiropractor near them. You are creating demand, not catching it, so TikTok behaves like Meta prospecting and not like Google Search. Expect longer decision cycles, more "I'll book later" leads, and a heavier reliance on follow-up. Clinics that run TikTok without an automated follow-up sequence lose most of what they pay for.
Two situations where we do not recommend TikTok Ads: towns under roughly 30,000 people, where the addressable audience exhausts in weeks and frequency climbs past 4, and clinics whose only offer is a full-price first consultation. TikTok audiences respond to a clear, low-friction entry point.
Organic posting builds the raw material: it tells you which of your videos people actually watch. Ads put money behind the winners and point them at your postcode. Running ads without organic testing means paying to find out which clips work; posting organically without ads means the clip that finally goes viral is watched by people three countries away.
Our rule: post three to five organic clips a week for four weeks, then promote only the clips with a completion rate above 40% and a comment thread that asks "where is this clinic?". Spark Ads (promoting the organic post itself, with its likes and comments attached) outperform the same video uploaded as a fresh ad in nearly every account we run, because the social proof is visible.
In TikTok Ads Manager, use the Lead generation or Website conversions objective, never Traffic or Video Views for a clinic that wants patients. Traffic campaigns produce cheap clicks from people who never book.
TikTok's location targeting is coarser than Google's. Use a custom radius around the clinic: 10 km in a city, 20–25 km in a town. Check the delivery report after two weeks and remove outer areas that produce leads but no visits; patients rarely travel more than 20 minutes for a first appointment.
Age: run 25–54 as the core group. Under 25 books cheaply and shows up poorly; over 55 is on the platform but converts better on Facebook. Split age bands into separate ad groups only when you spend over €1,500 a month, otherwise you fragment learning.
Interests: "Health & wellness", "Fitness", "Sports", plus behavioural targeting for people who interacted with health creators in the last 15 days. Skip narrow interests like "chiropractic": the audience is too small and TikTok's interest categories are inferred, not declared. Broad targeting with strong creative beats precise targeting with weak creative on this platform, every time.
Across our clinic accounts, four formats produce almost all booked first visits. Each should be filmed vertically, captioned, and cut to under 30 seconds.
Hooks matter more than production. The first second should show the thing that makes people stop: hands on a spine, a patient mid-reaction, a posture photo with a red line. Text overlays should name the city. Clinics that put the city in the first frame see lead quality rise, because people who cannot get to you scroll past.
The offer that converts on TikTok is a first visit with a clear price and a fixed number of slots this week: a postural check-up, a first consultation with assessment, a sports-injury screening. Discounts on ongoing care do not work; the audience has not decided it needs ongoing care yet.
Send traffic to a dedicated mobile landing page, not your homepage. One offer, the price, a 60-second version of your best video, three reviews, and a booking form or calendar above the fold. TikTok's own Instant Form is acceptable for lead volume but produces lower show rates than a page with a real calendar, because the lead has not chosen a time. We use the Instant Form only when a clinic's landing page loads slower than three seconds on mobile.
These are the ranges we see in chiropractic accounts after the first month, with the setup above. Clinic size, city and offer move every number, so treat them as a sanity check, not a promise.

The pattern to notice: TikTok leads are cheaper and show up less often. The cost per visit still wins when, and only when, follow-up is automatic. A lead that receives a WhatsApp message within five minutes and a reminder the day before shows up at Meta-like rates; a lead that waits for a callback the next morning usually does not.
Start at €30–40 a day, hold for 14 days without touching anything, then judge on cost per booked visit, not cost per lead. Scale by 20% every four days once the cost per visit has been stable for a week.
Install the TikTok Pixel through your tag manager and connect the Events API from your booking system or CRM so that the booked appointment, not the form submission, is the optimisation event. Optimising for form fills teaches the algorithm to find people who fill forms. Optimising for bookings, even with lower volume, teaches it to find people who show up. Pass the event with the lead's hashed email and phone so TikTok can match it after iOS attribution loss.
Report weekly on four numbers only: spend, leads, booked visits, attended visits. Everything else is noise for a clinic owner.
TikTok is a demand-creation channel that suits clinics with a strong visual offer and an automated follow-up system. If after 60 days your cost per attended visit is more than 1.5 times what the same clinic pays on Meta, move the budget. If your Google Ads campaigns still have unspent search demand in your city, fund those first: catching someone who is already looking will always beat convincing someone who was not.
For most clinics the right mix is Google Search for intent, Meta for volume, and TikTok as the third channel once the first two are saturated. Where TikTok earns its place is in cities where Meta CPMs have climbed past €15 and the clinic has a chiropractor comfortable on camera.
