ChatGPT Ads for Chiropractors: How to Reach Patients Inside the Conversation (UK & Spain, 2026)

On 24 August 2026 ChatGPT started showing ads in Spain and 30 other European countries. The UK had them since June. For a chiropractic clinic this is the first genuinely new patient-acquisition channel since TikTok, and it arrives with almost no competition: OpenAI says tens of thousands of advertisers are live worldwide, and in our checks of chiropractic-related conversations in Spanish and English, the sponsored slot is still empty most of the time. This is what we know after setting up QuiroAds' own campaign, what the rules are for health advertisers, and how a clinic should decide whether to be in that slot.

What launched, where, and who actually sees the ads

  • Timeline. OpenAI began testing ads with US users on 9 February 2026, opened a self-serve Ads Manager to US businesses on 5 May, reached the UK on 6 June, and announced on 18 August that ads would expand to 31 European countries including Spain, France, Germany and Italy, live from 24 August. Self-serve access for European advertisers followed at the end of August, in beta.
  • Who sees them. Only logged-in adults on the Free and Go plans. Plus, Pro, Business and Enterprise subscribers never see an ad. That is the right audience for a clinic: the person asking ChatGPT whether a chiropractor can help with their sciatica is far more likely to be a free user than a paying professional.
  • How the ad looks. ChatGPT answers the question normally. When the conversation has commercial context, a clearly labelled sponsored block appears below the answer with a headline, a line of text, an image and a link. OpenAI states that ads do not change the answer and that advertisers never receive the user's conversation.

The moment you are buying: not a keyword, a decision

The mental shift from Google Ads is the important part. On Google you bid on "chiropractor Valencia". On ChatGPT you describe the conversation you want to appear in, and the system matches your ad to the topic and intent of the chat, plus location, language and device, without third-party tracking across the web.

The conversations that matter for a clinic are the ones a patient has before they ever type a clinic name into Google:

  • "Is a chiropractor or a physio better for lower back pain that comes back every few months?"
  • "How much does a first chiropractic visit cost in Madrid and what happens in it?"
  • "My teenager has been told she has scoliosis, what are the options besides waiting?"
  • "I sit ten hours a day and my neck hurts by 4pm, what actually fixes this?"

Those are mid-decision moments. The person has a problem, is evaluating options and has not chosen a provider. A clinic that appears there with "First visit with postural assessment, €X, this week, 10 minutes from you" is speaking to someone one step earlier than any search ad reaches, and usually alone.

The health rules: what a chiropractic clinic can and cannot do

This is the part most coverage skips. OpenAI's ad policies, revised four times between March and August 2026, treat health advertisers differently from a shoe brand.

  • Placement. Ads are kept out of what OpenAI calls sensitive user contexts, which include conversations about a person's mental and personal health. In practice the ad appears when the conversation is commercial (comparing providers, asking about prices, looking for a clinic), not when someone is describing symptoms in distress. Plan your creative for the comparison moment, not the diagnosis moment.
  • Who may advertise. Health advertisers are approved one at a time through manual review, and OpenAI publishes a list of eligible health categories (it names dental services, hospitals and urgent care, medical testing, vision, health insurance and others). Chiropractic is not named in the reported lists we have seen, so expect to submit the clinic's licensing and a description of services and to wait for a decision. Approval through an agency that already holds an account has been faster in our experience with other platforms; on ChatGPT we are learning this in real time.
  • Claims. Unsupported treatment claims, experimental therapies and cure language are prohibited. "Fix your scoliosis" will be rejected. "Postural assessment and adjustment for back pain, first visit €X" will not.
  • Consent in the EU. Personalised targeting in the European Economic Area starts limited and consent-based under GDPR. Early European campaigns should assume contextual matching first and audience features second.

Two practical consequences: do not wait for approval to build the landing page and the offer, since that is the slow part on your side, and do not copy Meta ad copy with symptom lists into ChatGPT.

What it costs, and why nobody has a benchmark yet

In the US, where the platform has run since February, reported figures put clicks at roughly $3 to $5 and daily budgets from around $25, with CPM, CPC and conversion-optimised bidding all available. Europe opened on 24 August, so there are no local benchmarks; the first weeks are price discovery for everyone. Industry analysis of 50,000 commercial prompts in the US found a sponsored result on about 26 percent of them, and on about 29 percent in the healthcare category, which tells you the inventory exists and is still thin.

Our guidance for a single-location clinic: treat ChatGPT Ads as a test line, not a budget shift. €10 to €20 a day for six weeks, one offer, one landing page, conversion tracking on the booking, and judge it on cost per attended first visit against your Google and Meta numbers. If you are spending less than €900 a month on Google Ads with unspent search demand in your city, fund Google first; ChatGPT is a third channel, not a replacement. Setup checklist before you spend a euro

  1. Register at ads.openai.com and start the health advertiser review with your licensing documents. Do this first because it is the step you do not control.
  2. Install the OpenAI Pixel and connect the Conversions API to your booking system so the optimisation event is a booked appointment, not a page view. The pixel loads from an OpenAI CDN that strict Content Security Policies block silently; check the network tab, not just the tag manager.
  3. Write the conversation map. Twenty real conversations a prospective patient has, in the language they use, sorted by how close they are to booking. This replaces the keyword list and it is the brief for your ad matching.
  4. One offer, one landing page. A fixed-price first visit with a visible calendar. The landing page we describe in the patient acquisition guide works unchanged here.
  5. Answer within five minutes. ChatGPT leads arrive mid-decision and cool fast. If nobody can reply in five minutes, put an AI receptionist in front of the booking before you launch.
  6. Measure four numbers weekly: spend, leads, booked visits, attended visits. Ignore impressions and clicks for the first month.

Organic visibility in ChatGPT is a separate game

Paying for the sponsored slot does not change what ChatGPT says in the answer above it. When someone asks "best chiropractor in Málaga", the names in the answer come from what the model can find and trust: your Google Business Profile, your reviews, the pages on your site that describe services clearly, mentions on directories and local press. Who should start now, and who should wait

Start now if you are in a city with real ChatGPT usage (any Spanish provincial capital, any UK city), you already have a landing page and a follow-up system that work on Meta or Google, and you can put €300 to €600 a month into a test without touching your proven channels. Being early in an uncrowded auction is a real advantage that will not last: OpenAI reported an annualised revenue run-rate of roughly a billion dollars within six months of the US launch.

Wait if your Google Business Profile is incomplete, your reviews are under 30, or your last Meta campaign produced leads that nobody followed up. ChatGPT will not fix a leaky funnel; it will fill it with cheaper leads that leak just the same.
