Most chiropractors use ChatGPT for one thing: writing the occasional Instagram caption. Meanwhile their prospective patients use it for something else entirely: deciding whether to see a chiropractor at all, which one, and what it should cost. Since August 2026 there is also a paid way into that decision in Spain and the UK. Here are the seven ways ChatGPT moves the numbers of a clinic in 2026, ordered from free to paid, with what each one is worth and what it is not.
When someone in your city asks ChatGPT "who is a good chiropractor near me for a herniated disc", the answer names clinics. Those names come from sources the model can read and trust: a complete Google Business Profile with the right primary category, a steady flow of recent reviews that mention specific conditions, service pages on your website that say plainly what you treat and what a first visit costs, and mentions in directories and local press.
None of this is new work; it is the same local SEO that earns the map pack, now read by a second machine. What changes is the weight of clarity. A page titled "Our services" loses to a page titled "Chiropractic treatment for sciatica in Getafe: first visit, price and what to expect". The clinics we see cited most often are the ones whose website answers a patient's question in the first paragraph. The review system and the local SEO guide are the working list.
ChatGPT shows a labelled sponsored block under its answer when the conversation has commercial context. It has run in the UK since June 2026 and in Spain and 30 other European countries since 24 August 2026, for logged-in users on the Free and Go plans. Health advertisers go through manual approval and ads stay out of sensitive personal-health conversations, so the moment you buy is the comparison moment: "chiropractor or physio for recurring back pain", "how much is a first visit in Madrid".
The auction is still thin. In our checks of chiropractic conversations in Spanish and English the sponsored slot is empty most of the time. The same technology that powers ChatGPT now answers clinic phones. ChatGPT is a good writer's assistant and a bad chiropractor. Use it to turn one real explanation you recorded in the clinic into a caption, a short script, an email and a Google Business post; do not ask it to invent clinical claims. Export three months of appointments from your practice software, paste the anonymised table into ChatGPT, and ask the questions you never had time for: which weekday has the most no-shows, how many first visits become a third visit, what share of new patients came from each source, what a new patient is worth over twelve months. You will not get a consultancy report; you will get the two or three numbers that change how you spend on marketing next month. Strip names and contact details before you paste anything, and use a paid plan with training turned off.
Reactivation messages, pre-visit instructions, answers to the five objections every new patient has, the reply to a two-star review. ChatGPT drafts them in a minute; your job is to make them sound like your clinic. Give it three real messages you have sent that worked and ask it to match the tone.
