In Spain, a new chiropractic patient won through online advertising costs €72 on average. That figure comes from €170,921 in ad spend, 35,210 leads and 2,372 new patients at 35 chiropractic clinics, all of them QuiroAds clients, between October 2025 and August 2026. The result we didn't expect: in Madrid and Barcelona a lead costs less than in the rest of Spain, but a patient costs more.

Key figures

  • €72 in ad spend for each new patient who attends a first visit (median per clinic: €55).
  • €4.85 per lead, and 6.7% of leads become patients.
  • Up to 4 times difference in cost per patient between clinics: from €36 in the most efficient quartile to €143 in the least efficient.
  • Madrid and Barcelona: leads are 28% cheaper, but patients are 24% more expensive than in the rest of Spain.
  • Google converts almost 3 times better than Meta, but each patient costs 36% more.
  • Summer: a patient costs 80% more in July than in February.
  • 7 days: half of new patients attend their first visit within a week, and 90% within a month.

Cost per patient by platform

83% of the spend went to Meta Ads (Facebook and Instagram), 16% to Google Ads and less than 2% to TikTok Ads.

PlatformCost per leadCost per patientLead to patient
Meta Ads€4.29€696.2%
Google Ads€16.58€9417.7%
TikTok Ads*€4.48€726.2%
Total€4.85€726.7%

* TikTok: 42 patients in the period, which is a small sample. Treat it as a rough guide.

Google and Meta do different jobs. On Google, the person is already looking for a chiropractor, so a lead costs almost four times as much but is almost three times as likely to become a patient. Meta reaches people who were not searching yet. Its leads are much cheaper, and the clinic has to work harder to turn them into patients. In this data, Meta brings in the most patients and Google converts best. Neither one replaces the other.

Madrid and Barcelona: cheaper leads, more expensive patients

AreaCost per leadCost per patientLead to patient
Madrid and Barcelona€3.40€676.3%
Rest of Spain€4.70€5411.3%

Medians per clinic (22 clinics with at least €1,000 in ad spend and 10 new patients in the period: 6 in Madrid and Barcelona, 16 in the rest of Spain).

The two big cities have more people to reach, so leads cost less. But those leads convert at almost half the rate: 6.3% against 11.3%. A likely reason is choice. With more clinics nearby, more people ask for information and never book. Outside the big cities a lead costs more, but the person who gets in touch usually comes in. A clinic in Madrid or Barcelona will get more out of converting the leads it already has than out of buying more.

Clinics differ more than channels do

MetricMost efficient quartileMedianLeast efficient quartile
Cost per lead€3€4€7
Cost per patient€36€55€143
Lead to patient14.2%8.8%5.6%

A typical clinic in the most efficient quartile pays about a quarter of what a typical clinic in the least efficient quartile pays per patient. Same profession, same country. Much of that gap comes from what happens after the click. Lead-to-patient conversion ranges from 5.6% to 14.2%, and it depends on how fast the clinic replies, what it offers for the first visit and how it follows up. Our benchmarks guide has more on reply speed.

How the cost changes through the year

MonthCost per leadCost per patientConversion
October 2025€4.36€696.3%
November 2025€4.38€775.7%
December 2025€4.07€705.8%
January 2026€4.22€567.5%
February 2026€4.16€547.6%
March 2026€4.76€657.3%
April 2026€5.51€648.6%
May 2026€6.58€818.1%
June 2026€5.72€846.8%
July 2026€5.25€975.4%
August 2026€4.48€905.0%

January and February are the cheapest months to win new patients (€54 to €56), with the highest conversion of the winter. July and August are the most expensive (€90 to €97). The cost per lead rises much less. What changes is that far fewer people book during the Spanish summer holidays. If your budget is limited, putting it into January to April gets you more patients than spreading it evenly across the year.

How long a lead takes to become a patient

  • Median: 7 days from the moment the person leaves their details to their first visit.
  • 53% come in within the first week and 90% within the first month.
  • By platform: 7 days on Meta, 8 on Google and 14 on TikTok.
  • At least 3 in 10 new patients go on to start a care plan.

This has two practical consequences. First, judge a campaign after four weeks, not after three days. Second, when a clinic marks a lead as a new patient, the value it records has a median of €50. That matches the usual price of a first visit in Spain, so the first visit on its own does not pay back what it cost to win the patient. The return comes from patients who carry on with care. That is why follow-up and automation matter as much as the ads.

Methodology

  • Sample: 35 chiropractic clinics in Spain with active campaigns managed by QuiroAds. The data is aggregated and anonymized, and no clinic can be identified.
  • Period: leads created between 1 October 2025 and 31 August 2026, and ad spend over the same period. September 2026 is left out because recent leads have not had time to come in yet.
  • Lead: a person recorded in the clinic's CRM from a Meta, Google or TikTok ad. Duplicates are removed (one per person per clinic).
  • New patient: a lead who attends a first visit, as recorded by the clinic in its own CRM.
  • Ad spend: the actual amount spent on each platform. All amounts in this report are in euros (€), the currency the clinics paid in.
  • Clinic comparisons: medians and quartiles of the 22 clinics with at least €1,000 in ad spend and 10 new patients in the period.

Limitations. A patient only counts if the clinic records them in its CRM, so some visits may be missing. This makes the figures conservative, and the real cost per patient may be a little lower. Google figures do not include phone calls that never reach the CRM. These figures are for chiropractic in Spain, and they measure the cost up to the first visit. They can't be compared directly with other healthcare specialties (we explain why in our guide to marketing for clinics, in Spanish) or with the planning ranges in our benchmarks guide, which are estimates, not measured data.

Frequently asked questions

How much does it cost to get a new chiropractic patient in Spain?
On average, €72 in ad spend for each patient who attends a first visit, based on data from 35 chiropractic clinics between October 2025 and August 2026. The typical clinic (the median) pays €55, but the range runs from €36 at the most efficient clinics to €143 at the least efficient.
Which is better for a chiropractor, Google Ads or Meta Ads?
It depends on what you need. In this data, Meta brings in cheaper patients (€69 against €94) and most of the patients overall, while Google converts almost three times better (17.7% against 6.2%). Most clinics use both: Google for people who are already searching and Meta for volume.
Why does a new patient cost more in Madrid and Barcelona?
Leads there are cheaper (€3.40 against €4.70), but they convert at almost half the rate: 6.3% of leads become patients, compared with 11.3% in the rest of Spain. There are more clinics to choose from, and more people ask for information without booking.
What is the best month to advertise a chiropractic clinic?
In this data from Spain, January and February, with a cost per patient of €54 to €56. July and August are the most expensive (€90 to €97), because far fewer people book during the summer holidays, while the cost per lead rises much less.
Do these figures apply to other specialties?
Not directly. This is chiropractic data from Spain. Cost per patient varies a lot between specialties because the price of a click, the value of a patient and the time people take to decide are all different. We explain this in our guide to marketing for clinics (in Spanish).
